First Gout Flare – Help!
Hi Dmitry,
I have been on all variations of the protol now for over about a year, averaging about 3 grams a day of Niacin and respective others and over the past 2 weeks have encountered my first ever gout flare, also appears to be some Tophi developing in my left big toe which rapidly grew over about 2 months.
It has been quite a severe attack that seemed to have developed out of nowhere and I am quite puzzled as to how I developed gout + tophi so rapidly as I dont eat hardly any of the trigger foods (except I eat a small portion of red meat once a week due to having hemochromoosis) as I am allergic to gluten and dairy which rule out most sweet and processed foods. I also have never drunk alcohol as I have had a fatty liver that I became aware of in my early 20’s (now am 42).
Previously my highest single dose ever of Niacin was about 7 grams on another variation of the protocol, however I have never been able to go above about 3 grams since the addition of l-glutamine and n-acetyl-l-glutamine as i become quite naseous and vomit about 6 hours later.
I can see from some of these threads your recommendation for gout is to go higher with Niacin + NAG, though I dont know how I can achieve this as I will vomit well before I get there.
I have currently paused the protocol until I can get a baseline of my urate levels. Any advice you could provide would be sincerely appreciated as this is extremely uncomfortable and all DR’s are throwing gout meds at me which I have refused because I would like to some how manage it with the protocol.

This is certainly discouraging to hear, albeit the worst of it should be over now. With individual powders, not having all three nutrients (and not just one dose but as you see for weeks continued) consistent can lead to unpredictable exacerbations like this. This is what makes the https://GPR109A.com already worth it, given niacin, folic acid, and N-acetyl-L-glutamine are precisely fused with no hassle but also now given that we will not have to try to exhaust dosage so much as we were without this key IRON CITRATE I mention in the most recent post on our HOM3OSTASIS feed here.
I do believe with ANY history of nagging gout, especially with an out-the-blue flare-up like this, mandates precisely-tuned application and most especially, consistently whilst trying to minimize variance (and given the relative mass of folic acid being so minute, we are asking for something to go wrong as the body remodulates (and this is in fact one major reason Pharma makes drugs so the patient does not have to prepare it himself). GPR109A product linked above (or here directly: https://purebulk.com/products/GPR109A?sca_ref=1004090.8JspdBHd04) should massively circumvent this caveat. Furthermore, adding the Swanson iron citrate per the newest post as spot-therapy alongside, which should simultaneously really ease having to chase doses of GPR109A with the proper iron citrate coming now. Meanwhile, combining 1-2 capwules of the proper-linked iron citrate to recoup it as per my newest post), the GPR109A individual dose sizes, even in the worst-case scenarios, I feel, will remain no more than 4 g niacin anchors (with the other two in proper ratio), with response coming smooth, rapid as it has been the case with all three per GPR109A for those less iron deficient.
The dysregulation of iron is a common feature then that leaves iron in cells no longer staying in flux through and so accumulating and oxidizing. This iron overload isolated to certain cells or regions of the body has nothing to do with what actually delivering (the proper) iron (with its identified factors alongside) as we are here will be doing. In fact, herein, the purpose of the iron citrate spot-boost/therapy is to restore the deficiency systematically through the body that led to per se, iron overload in individual cells/organs (literally left more to die with the limited iron going to cells more favorable). We literally have EVERYTHING but the actual iron substrate … which in fact I had (iron) along with folic and niacin.. just not N-acetyl-L-glutamine as now but selenium. Please let me know if this makes sense so far…. (as well as my above newest post on this).
